HEMATOMA INTRAPARENQUIMATOSA PDF

An intracerebral hemorrhage , or intraparenchymal cerebral hemorrhage , is a subset of an intracranial hemorrhage. This can encompass a number of entities that share the acute accumulation of blood in the parenchyma of the brain. With any intracerebral hemorrhage the following points should be included in a report as they have prognostic implications 3 :. Brazilian guidelines for the manegement of intracerebral hemorrhage. Among the stroke subtypes, intracerebral hemorrhage ICH has the worst prognosis and still lacks a specific treatment. The present manuscript contains the Brazilian guidelines for the management of ICH. It was elaborated by the executive committee of the Brazilian Cerebrovascular Diseases Society and was based on a broad review of articles about the theme. The text aims to provide a rational for the management of patients with an acute ICH, with the diagnostic and therapeutic resources that are available in Brazil.

Metrics details. Despite significant progress in the acute management of these patients, the ideal surgical management is still to be determined. Surgical hematoma drainage has many theoretical benefits, such as the prevention of mass effect and cerebral herniation, reduction in intracranial pressure, and the decrease of excitotoxicity and neurotoxicity of blood products. Several surgical techniques have been considered, such as open craniotomy, decompressive craniectomy, neuroendoscopy, and minimally invasive catheter evacuation followed by thrombolysis. Open craniotomy is the most studied approach in this clinical scenario, the first randomized controlled trial dating from the early s.

A simple categorization is based on location:. Alternatively, intracranial hemorrhage can be thought of in terms of the underlying cause, although in most cases the same etiology can result in multiple different patterns of hemorrhage. CT scan is almost always the first imaging modality used to assess patients with suspected intracranial hemorrhage. Fortunately acute blood is markedly hyperdense compared to brain parenchyma, and as such usually poses little difficulty in diagnosis provided the amount of blood is large enough, and the scan is performed early. MRI is typically requested when an underlying abnormality is being sought, particularly when an underlying tumor is suspected. MRI of hemorrhage can pose some challenges in that the appearance of blood changes depending on the sequence and the time since the hemorrhage and the size and location of the bleed.
